    Affidavit Request for Exemption (NOTE: It may take up to 3 weeks for your request to be processed by the State). From the Texas Department of State Health Services: "Exclusions from compliance are allowable on an individual basis for medical contraindications, active duty with the armed forces of the United States, and reasons of conscience, including a religious belief."

    If your child's healthcare provider is unable to provide vaccinations, then here are some resources (call them first to make sure they have the vaccinations in stock and will administer them to your child based on their age). Some clinics or pharmacies may require you to bring in a current vaccination record and a list of what vaccinations they are missing:
